The best-rated hand cream on Amazon costs $8.75. O’Keeffe’s Working Hands had 83,192 ratings at 4.7 stars when we read it on 26 September 2026, with 93 per cent in the four-to-five band. Nothing upmarket comes close to that record.
So every pick on this page is measured against that jar. Where a $25 tube beats it, we say how. Where it doesn’t, we say that too.
What we found is that the premium creams rarely fail on moisture. They fail on smell. The complaint that turns up on every scented pick here is the fragrance: 16 of 42 critical reviews for Jack Black, 10 of 32 for Kiehl’s, 10 of 21 for L’Occitane, 9 of 31 for Aesop and 8 of 27 for Brickell. That is what the extra money buys.
Which of these is legally a drug
A hand cream that claims to protect or heal chapped skin is making a drug claim. FDA’s skin protectant monograph, 21 CFR 347.10, lists the actives that can carry that claim. They include dimethicone at 1 to 30 per cent, glycerin at 20 to 45 per cent, petrolatum at 30 to 100 per cent, and allantoin. A product that relies on one must declare it in a Drug Facts panel and be listed with FDA.
We searched FDA’s National Drug Code directory, through FDA’s own openFDA interface, for every cream on this page on 26 September 2026. None of the six ranked picks is listed.
Two of the drugstore jars are more interesting. Gold Bond’s Healing Hand Cream has 40,826 ratings and the word healing in its name. It isn’t in the directory; the Gold Bond hand cream that is listed is the Eczema Relief version, with 2 per cent colloidal oatmeal.
O’Keeffe’s plain Working Hands isn’t listed either. The O’Keeffe’s product registered as a skin protectant is the Pain Relief version, pairing 0.5 per cent allantoin with 4 per cent lidocaine.
The only cream on this page listed as a skin protectant is CeraVe Therapeutic Hand Cream, with dimethicone at 10 milligrams per gram. That is 1 per cent, the bottom of the permitted range.
None of this makes the cosmetics worse. A cosmetic can moisturize perfectly well; it just can’t claim to treat anything. The directory records what companies list, and a product missing from it may be sold as a cosmetic by design. We didn’t test anything.
What the critical reviews complain about
Amazon’s star filters work when you are signed in. On 26 September 2026 we read every critical review the one-, two- and three-star filters served for each cream, removed duplicates and free-product reviews, and sorted them by hand.
The unscented creams, La Roche-Posay and O’Keeffe’s, draw almost no scent complaints at all. Their critics object to texture and to results instead. That is the trade on this page in one table.
The drugstore jars, for the record
These are not ranked because this page is about the upmarket end. They belong on it anyway, because on data alone every one of them matches or beats the picks above. Figures read 26 September 2026.
If you don’t care how a hand cream feels or smells, stop here. O’Keeffe’s Working Hands has the biggest record in the category and no scent complaints at all among the 39 critical reviews we read.
Its critics say it is sticky or didn’t do enough. A June 2026 one-star: “I work in automotive. This is sticky even if applied after a shower and left to dry overnight.”
Gold Bond Healing is the cheapest per ounce by a distance. Neutrogena Norwegian Formula is the old glycerin standby in a two-ounce tube. Eucerin Advanced Repair uses urea and comes in a three-pack. CeraVe Therapeutic Hand Cream is the only one here listed with FDA as a skin protectant, and it also carries the National Eczema Association’s Seal of Acceptance, checked the same day.
What the extra money buys
Feel and fragrance. The upmarket creams absorb differently and smell of something. Their buyers are as satisfied as the drugstore buyers, not more, and their critics mostly object to the smell.
That is a fair thing to pay for. It is not the same as paying for results.

Buyer sentiment first, with review count weighted ahead of review average, and a floor of 4.1 stars, more than 100 ratings and at least 60 per cent of ratings in the four-to-five band. All six picks clear it. La Roche-Posay ranks first on sample size; Kiehl’s has the best distribution but a record under half of Jack Black’s.
Every figure was read from the Amazon listings in a logged-in session on 26 September 2026. The complaint counts are ours, sorted by hand from the reviews Amazon served, which Amazon caps per filter. The FDA listing checks used openFDA’s National Drug Code endpoint the same day; FDA notes that listing is not approval.

Is O’Keeffe’s Working Hands a drug?
No. When we searched FDA’s National Drug Code directory on 26 September 2026, the plain Working Hands cream was not listed. The O’Keeffe’s product listed as a skin protectant is the Working Hands Pain Relief version, with 0.5 per cent allantoin and 4 per cent lidocaine. The plain cream is sold as a cosmetic.
Which hand creams are FDA-registered skin protectants?
Of the eleven creams on this page, only CeraVe Therapeutic Hand Cream, listed with dimethicone at 1 per cent. We checked the others, including Gold Bond Healing Hand Cream, and found them absent from the directory. Being a cosmetic doesn’t mean a cream moisturizes less, only that it can’t claim to treat anything.
Why do expensive hand creams smell so strong?
Fragrance is much of what distinguishes them from drugstore creams, and it is also what their critics object to most. In the reviews we read, scent was the leading or second complaint for every scented pick here. If you want an upmarket feel without the smell, La Roche-Posay’s unscented cream is the closest.
Is hand cream different from body lotion?
Usually, yes. Hand creams are thicker and more occlusive, because hands are washed many times a day and lose moisture faster. A body lotion will do in a pinch, but tends to wash off sooner. Apply hand cream after washing and again before bed, when it has hours to work.
